This application requires the following narratives or uploads, and cannot be saved prior to submission. Incomplete applications will not be accepted.
  1. Applicant’s business plan
  2. Applicant’s abstracting experience
  3. Three (3) abstracts prepared by Applicant
  4. History of any professional disciplinary actions
  5. Facts that justify a waiver for the Applicant under IAC 265—9.7(1) “d”(5) and 9.7(1)”d”(6)”4” Download Rule Sections

Tips for Uploading Documents
  1. Only .pdf format documents may be uploaded.
  2. Do not combine multiple required documents as one file. Each required document should be saved and uploaded as an individual file.
  3. The maximum file size for the total of all uploaded documents is 150 MB.
  4. Check your file sizes in advance. The combined file size for the application submission cannot exceed 150MB. To determine the size:
    1. Locate the folder on your computer that contains your documents.
    2. Select the Details option from within the "Views" drop-down menu.
    3. The file size displays in the "Size" column.
    4. Add the file sizes for all of the documents to be submitted. If your documents are sized in KB, there are 1,000 KB in 1 MB.
